Surgical Procedures
Surgical procedures on ovaries
a. Ovarian biopsy may be necessary to confirm or identify suspicious lesions. Therapeutic multiple biopsies, wedge resection, or ovarian drilling with electrocoagulation have been recommended for patients with polycystic ovarian disease that is resistant to standard ovulation-induction regimens.
b. Oophorectomy is a surgical removal of pathologically changed ovary
c. Laparoscopic ovarian cystectomy is reserved for cysts with no features suspicious for malignancy; solid tumors or tumors with a solid component can be carefully evaluated through the laparoscope, and the procedure can be converted to a laparotomy when appropriate.
Surgical procedures on fallopian tubes
a. Salpingo-ovariolysis is an adhesiolysis involving tubes and ovaries.
b. Phimotic or clubbed fimbriae are one of the causes of infertility due to mechanical closure of ovarian end of tubes. Fimbrioplasty and salpingostomy is a reconstructive surgical procedure on the fimbrial end of tubes.
c. Salpingectomy is the treatment of choice for an ectopic pregnancy when preservation of fertility is not an issue, or when the fallopian tube has been markedly damaged by rupture or adhesive disease.
Myomectomy
Procedure Description
Some women with symptomatic myomas may prefer selective removal of myomas to hysterectomy because thay want to preserve fertility or retain the uterus. In properly selected cases, laparoscopic myomectomy may be feasible. Myomectomy can be more difficult and time consuming than hysterectomy, and can be associated with greater blood loss.
